Root Cause Analysis in nursing student’s clinical setting
To develop an understanding of the root cause of the errors by students in a clinical setting, the Department of Family Health at the University of San Francisco conducted a Root Cause Analysis (RCA). Four questions were explored and answers analysed. As a result of the RCA three important needs were identified and a full report was shared with the managers of the unit in which the errors occurred.
